1. what the document asking for is you must report to some health clinic right after landing, then health ministry can monitor your wife's health status. no matter what happens later, you two still keep PR status in Canada.
2. It's not asking for another medical exam in Canada. If I were in your shoes, I'd do the following..
* After landing in Canada, report to local health ministry office ASAP.
* From list of authorized medical exam clinics in Canada, find out the one which is the most convenient for you, pay a visit there after you have SIN number.
* Inform local health ministry office you have done what they ask for.
3. Leaving Canada after you have your PR card, it amy take 4-8 weeks, either you or your wife better stay a little longer to make sure no trouble with PR cards.
