This is in reference to your application for permanent residence in Canada. Please send the following
documents and RETURN THIS FORM WITH THE REQUESTED DOCUMENTS.

o ORIGINAL passport for you and all your accompanying dependants
o carry out our medical instructions at one of our designated medical practitioners, for you and your
dependants – Please present this letter to the doctor as well as your original passport. Upon
completion of the examination you will be provided with proof that you have taken the
examination, you need to return this proof to the embassy with your original passport.
If you are currently not residing in China and will not undergo the medical
examination in China, please advise us immediately so we can provide you with
different instructions.
o ask sponsor to pay Right of Permanent Residence Fee in Canada (RPRF) of CAD $490 per person,
for you and your accompanying spouse (if applicable) if not paid already.

Please complete the copy of the RPRF receipt with the sponsor’s information and send it to Case
Processing Centre Mississauga (CPC-M) by e-mail to CPCM-EXTCOM@cic.gc.ca. Please visit the

apply-after.asp
If you do not have an email account, please send the RPRF receipt to CPC-M by fax or mail.
Completed Schedule 1 form (enclosed), for all applicants over the age of 18.
(Please complete all fields from your 18th birthday and until today, for any section that does not
apply to you, please write “not applicable”)

oAll documents must be accompanied by English or French translations. When ORIGINAL documents are
requested, notarized documents are not acceptable.

We recommend that you submit all original documents using registered mail. Please note that when you
send us an EMS or other registered mail you should clearly record the registered mail number, and if you would like confirmation that the mail has arrived at the Embassy you can contact your postal service. Our office is unable to provide confirmation for each document delivered.

Be advised that the Embassy is not responsible for any negative consequences arising from your
passport application. It is also your responsibility to comply with PRC exit regulations.

Please ensure that your file number, and name & address in Chinese characters is included with all
correspondence. Please mail the requested documents within 45 days. ALL correspondence with the
