我想请教各位.为孩子做出生证时:

603207(咪咪)
有一项是Fee and Payment
first birth certificate (includes basic information,such as name ,date and place of birth).--------------------------------------$25
replacement birth certificate.----------------$35
first certified copy(contains all registered information,including parent's information and signatures)---------------------$35
replacement certified copy of birth---------$45
confirmation letter(a letter saying the birth is on file)--------$15
我该选取哪些呢?全部都有选有必要吗?谢谢!

      1,在出生证申请表的第一栏,Applicant's Name是我的名字还是我孩子的名字?
      2,如果没有认识两年的guarantor,第五项Statement of Guarantor是否就空在那里?
