We are pleased to announce that the ATIP Division has developed a new consent form, which is to be used when making an ATIP request. It is easy to fill out and better corresponds to your needs.

→ Consent for an Access to Information and Personal Information Request - IMM 5744

(http://www.cic.gc.ca/english/department/atip/form-imm5744.asp).




As of February 4, 2013, we will no longer be accepting the following consent forms for ATIP purposes:
→ Authority to Release Personal Information to a Designated Individual - IMM 5475,

→ Use of Representative - IMM 5476.



You can access the new ATIP request form at the following link:
http://www.cic.gc.ca/english/department/atip/form-imm5563.asp.

2) Submit your ATIP Request Electronically

In order to submit your request by e-mail, you must:
→ Complete, print, sign and scan your request form; and

    请问有经验的同学，父母移民有明确的结果是因为体检不合格的吗？还是会让老人们继续复检，也不出明确结论。体检一年早过期了，久等无信，前二天来通知要母复检。更令人不解的是，要求父可以在母的复检被审查后再去重新体检！请问有人遇到类似的复杂情况吗？为什么会这样要求？
