1) New Consent & ATIP Request Form

→ Consent for an Access to Information and Personal Information Request - IMM 5744

(http://www.cic.gc.ca/english/department/atip/form-imm5744.asp).

As of February 4, 2013, CIC will no longer be accepting the following consent forms for ATIP purposes:
→ Authority to Release Personal Information to a Designated Individual - IMM 5475,

→ Use of Representative - IMM 5476.

You can access the new ATIP request form at the following link:
http://www.cic.gc.ca/english/department/atip/form-imm5563.asp.

2) Submit your ATIP Request Electronically

In order to submit your request by e-mail, you must:
→ Complete, print, sign and scan your request form; and
→ Pay the $5.00 application fee by credit card through CIC’s online payment service (for ATI requests only). You can access e-payment here: https://eservicesak.cic.gc.ca/epay/welcome.do?lang=en.

    • 2012年3月体检如果还没拿到移民纸，一定要写email给大使馆去问（一般不会答复），还要不停的问MP。我的经验是，你不能嫌麻烦，把email写得感人点儿，MP都是挺乐意帮的。我从去年10月一共问了MP四次，因为父亲背景调查耽误了时间，上个月终于拿到了。祝你好运。 -teletubby(天线宝宝); 2013-9-25 (#8392727@0)

          我也是壮着胆子把护照借出来，让他们用以前申请的多次往返签证先过来的，然后把地址改成了这边儿。还没借出来一个月，加拿大北京使馆就发email让我把护照还回去（背景调查可还没结束呢）。我又写email去问MP该怎么办, MP说是还护照，让我一个月之后没消息，再找她问。我只好把护照寄回去（并回了使馆一封信，说明不能多等，因为人在这边呢），过了一个月又写信给MP去问，这才告诉我背景调查结束了，但还要经移民官审查才会有结果。不久我又收到使馆email让交前苏联的police clearance。交了之后大概一个月，MP打电话给我说好消息。我已经谢天谢地了，没有让我们回北京去面试。总之，不要怕麻烦，让移民局知道你们很着急一直在等结果。就当是好事多磨吧。

      My parents' case is in the similar period as yours. They were asked to redo medical check and background check. But we didn't get the mail. The communication channel with Ottawa is not working. Something like one to two monthes reply your email if you are lucky.

      My MP Paul's office did a lot of help. They follow up with Ottawa every 2 to 3 monthes even the MP assisstant lady changed in the middle. In this Oct, after 1.5 years since their first medical exam, my parents get the big envelope. I would say without the communication between my MP's office with Ottawa, I will finally lose my patent.

      So I would suggest try to looking help from your MP's office.
